Rectangle & Ellipse Tool Troubles CS6

i discovered following (taken cs6 file) no longer seems work. 85 minute call adobe left me no further ahead. it's entering width value , clicking on "height" copy width value height field. same behaviour rectangle tool.

 

do 1 of following:

• drag diagonally until ellipse desired size.
• click want top-left corner of ellipse’s bounding box be. specify width , height ellipse, , click ok.
• note: to create circle, hold down shift key while dragging, or if specifying dimensions, once you’ve entered width value can click on word height copy value height box.

just clear, having trouble when try click once using rectangle or ellipse tool , enter in values?

 

 

just right of width and height fields there small icon of chain-link. appear broken (left image) or connected (right image)?

 

also, helpful know if attempting create perfect circle/square or if trying create shape different width , height values?
